Med-legal billing: ML200–ML205 without leaving money behind
The medical-legal fee schedule, modifiers, excess pages and testimony.
Guide
The schedule
Medical-legal evaluations are billed under the Medical-Legal Fee Schedule in 8 CCR §9795(c), codes ML200 to ML205, plus ML-PRR for excess record pages.
The codes
ML200 — missed appointment for a comprehensive or follow-up evaluation ($503.75), including a cancellation within six business days of the appointment.
ML201 — comprehensive medical-legal evaluation ($2,015). “The fee includes review of 200 pages of records. Review of records in excess of 200 pages shall be reimbursed at the rate of $3.00 per page.”
ML202 — follow-up evaluation within eighteen months by the same physician ($1,316.25), including 200 new pages.
ML203 — supplemental evaluation ($650), including 50 pages; not allowed for information that was already available before the prior report.
ML204 — testimony ($455/hr, or usual fee if less), by the quarter hour, including reasonable preparation and travel; “a minimum of two hours for a deposition,” and a minimum of one hour if the deposition is cancelled fewer than eight calendar days before.
ML205 — review of sub rosa recordings ($325/hr, or usual fee if less), by the quarter hour, with time verified in the report.
All figures quoted from 8 CCR §9795(c) as published on 17 October 2026.
Where money is left behind
Excess pages. Every page over the included allowance is billable at $3.00 under ML-PRR — but only with “a verification under penalty of perjury of the total number of pages of records reviewed” in the report.
Modifiers. -93 (interpreter or impaired communication, ML201/ML202) multiplies the value by 1.1; -94 (Agreed Medical Evaluator) by 1.35, or 1.45 with -93 (§9795(d)).
Testimony and cancellations. The two-hour deposition minimum and the one-hour late-cancellation minimum are easy to forget when time is logged by hand.
Late payment. Medical-legal bills unpaid after 60 days carry a 10% increase plus 7% interest (Lab. Code §4622(a)(1)).
How ChironAI™ OM helps
Attesting a med-legal report drafts the ML200-series charge with excess-page pricing; completing or cancelling a deposition drafts the ML204 charge; sub rosa review drafts ML205 — each for a biller to confirm. Try the ML fee calculator.
